Social inequalities in health – In Norway? Still?

What causes social inequality and how can we lift everyone to a good level of health?

One would be inclined to think that a welfare state like Norway which has grown rich on oil revenues would not suffer with social inequalities in health. But the facts tell us otherwise. There seems to be a persistence in the existence of social inequalities in health that is hard to explain and hard to change. Or is it? What do we know about the causes of these inequalities, and what is needed to lift the worse-off to a level of better health like those with higher socio-economic status?
